It's painful because the doctor may need to cut your gums or bone to get to the teeth. People who get their wisdom teeth removed are sedated before surgery, so most people have no pain after, except a mild discomfort during the next three days. People react different to anesthesia, some of them still feel drowsy after surgery, so that's why they look like drunk.
